§ 11-18. Notification of changes; updating applicant information.

  • (1)

    Every massage parlor, massage school, or bathhouse permittee shall report immediately to the occupational license inspector any and all changes of ownership or management of the massage or bath business, or massage school, including but not limited to changes of manager or other person principally in charge, stockholders holding more than ten (10) per cent of the stock of the corporation, officers, directors and partners; any and all changes of name, style or designation under which the business is to be conducted; and any and all changes or transfers of massage or bath technicians employed in the business whether by new or renewed employment, discharge or termination, or otherwise, and any changes in information required by section 11-7 of this chapter.

    (2)

    Every massage or bath technician shall report immediately to the occupational licensing inspector any and all changes of employment, whether by new or renewed employment, discharge or termination, or otherwise, giving the name and address of the former employee, if any, and any changes in the information provided under section 11-11 of this chapter.
